Make A Reservation
To make a reservation online, input the required information below and then click the PayPal/Pay Now button below or, mail or fax a completed enrollment form along with your check, money order or credit card information. Once we receive your reservation, we will confirm your registration subject to availability and send you a medical and personal history form to complete.Registration And Cancellation Policy
If you cancel your registration prior to April 1st, a full refund less a $250 registration fee will be given. No refunds are provided for cancellations less than 4 weeks in advance of the enrolled program start date. If you cancel more than 30 days prior to the start of the session in which you are enrolled, a full refund will be made of all fees previously paid, less a $500 registration fee. If cancellation is due to non-payment of tuition or within 30 days of the start of the session in which you are enrolled, all money received by Shane Diet Resorts shall be nonrefundable.
